This book, co-badged by BP and the Center for Chemical Process Safety (CCPS), describes about 50 incidents that have had a significant impact on the chemical and refining sector's approaches to process safety.rnEvents are described in some detail to allow a fundamental understanding of the consequences, root causes, lessons learned, and generic actions that can prevent incidents from recurring. These actions are recorded under standard headings such as: Hazard evaluation and management; Major accident potential; Protective systems; Management of change; and Competent personnel and procedures, thus providing a reference structure for assessing your own organisation, if desired.
