Prescription is an order of scientific medication for a particular patient at a particular time which forms a transaction amongst the Physician-Pharmacist- for a rational treatment. Rational Prescription is defined as prescription in particular patients receiving medication appropriate to their clinical needs in doses that meet their own individual requirements for an adequate period of time and lowest cost to them and their country. Evidence-Based Prescription means translation of evidence - based drug and therapeutic information into written and spoken advices to the patients. It estimates the physician's ability to locate critical information from the current medical literature and his/her willingness to incorporate patient's relevant values in decision making process.
