In ophthalmology, modern treatment approaches do not necessitate lengthy inpatient stays. Traditional processes therefore have to be adapted accordingly. Since changing processes always entails an element of risk, the change process was evaluated before its implementation by using computer simulation. Applying an engineering process using the systems engineering concept made it possible to collect all core processes of the department and to adapt these to the new requirements. In January–April 2007, compared with the same time period in 2006, the implemented measures resulted in a reduction in the average length of inpatient stays by 26.8%; the standard per-case (LDF Leistungsorientierte Diagnose-Fallgruppen) reimbursement, featured as a "price for points" depending on initial diagnosis, per discharge rose by 1.6%; and the LDF points per day rose by 40.5%.
