Six Canadian Armed Forces (CAF) personnel died April 29 when their Sikorsky CH-148 Cyclone anti-submarine warfare helicopter abruptly nosed down into the Ionian Sea (between Sicily and Greece) as it approached the Halifax-class frigate HMCS Fredericton to conduct pre-landing hoist training. The CH-148 fleet is now under an operational pause. A US Navy-CAF team completed recovering the wreckage in early June. According to the CAF, during its final maneuvering turn at low altitude to close with the ship, the aircraft did not respond as the crew would have anticipated. This aircraft entered a high energy descent and impacted the water astern the ship. Investigators are looking at software systems and human factors in their probe. (Source: CBC)
