The great 18* century psychiatrist Philippe Pinel (1745-1826) wrote: "it is an art of no little importance to administer medicines properly: but, it is an art of much greater and more difficult acquisition to know when to suspend or altogether to omit them". All good prescriptions must come to an end, and from the moment it is first prescribed, withdrawing the drug must be in the doctor's thoughts and objectives.How many patients come to our consulting rooms or wards with too many prescriptions for useless drugs? In such cases, our duty as doctors is to think about the harm-benefit balance of each of the drugs, as well as interactions and adverse effects. We must then eliminate unnecessary or harmful prescriptions, or in other words, "deprescribe".
