TO THE EDITOR: Horowitz et al. describe an instrument apparently designed to ask all hospitalized medical patients if they, are currently or, have recently, thought about killing themselves and if they have previously attempted suicide. Their study "was determined to be 'exempt' from institutional review board review," placing a heavier than usual onus on the Journal's reviewers and correspondents to assess its quality and worth. There are a myriad of problems with the study, 3 of which we highlight below. First, the whole premise of the "screening tool" rests upon the assumption that a hospitalized medical patient, who voices recent suicidal ideation or who has previously attempted suicide, is at increased likelihood of suicide in near future.
