The Long awaited results of the CHOICE (Choices for Healthy Outcomesin Caring for ESRD) Study have now been published, accompanied by a very thoughtful editorial (1,2). The CHOICE Study was-a collaboration between the Dialysis Clinic, Inc. (DCI) nonprofit chain and the investigators at Johns Hopkins University, with the addition of peritoneal dialysis (PD) patients from the Yale-New Haven Group (Alan Kliger and associates). This study was begun a decade ago. The investigators collected a host of important and pertinent data near the start of dialysis in PD and hemodialysis (HD) patients, and then followed the outcome. They found that the risk of death is increased on PD relative to HDafter the first year on dialysis. This is in contrast to previous studies from Canada and Denmark that show a survival advantage of PD over HD for the initial. 1.5 - 2 years of dialysis (3,4). We need to examine why the resuLts of this study are different.
