THE importance of medication safety, while long recognised by healthcare practitioners involved in any aspect of medication management, was underlined when in 1995 the Australian Quality in Health Care Study (AQHGS) was released to an incredulous Australian public, who learned for the first time that 16.6 per cent of patients in hospital were involved in injuries and complications arising from their treatment.Many of these incidents involved medications and it was realised that no matter how conscientious, careful and experienced individual practitioners were, the potential for human error existed.Unfortunately, it used to be the case that when an error occurred, the focus of any subsequent action was on who was to blame. However, there has been a maturing within the healthcare system. Now, ascertaining who was involved and who was to blame is seen as being far less important than discovering what, why and how it went wrong and what could be done to minimise the risk of any similar future occurrence.
