A patient known to have both organic seizures and pseudo-seizures failed to make a normal recovery after general anaesthesia. The recovery period was complicated by two clonic seizures. The decision to send him back to the ward was assisted by observing breath holding when his airway was deliberately obstructed and by information from a Bispectral anaylsis monitor which demonstrated readings between 92-96 throughout the pre, peri and post seizure period. This clinical test and the use of Bispectral analysis may help distinguish organic from pseudo seizures in situations where more sophisticated investigations are unavailable.
