Microliposuction and fat transfer for advanced repair of liposuction defects

Liposuction has become the most commonly performed cosmetic surgery in the United States. Because of the sheer numbers of procedures being performed annually by practitioners from multiple specialties with a wide discrepancy in training and approach, one would expect to see some unsatisfactory results requiring secondary procedures. Even the most expertly performed liposuction can potentially leave irregularities and depressions (Table 1). I have treated patients unhappy after liposuction who do not want to return to the original surgeon for obvious reasons. These patients are often difficult to satisfy based on their previous experience and unrealistic expectations. Some of the resultant defects such as post-overaggressive contouring may not be significantly improved or may require multiple procedures for maximum correction. However, many of these problems are amenable to repair even when an entire area is uneven, using a combination of liposuction done both by hand and with an aspiration machine along with fat transfer with or without subcision. Fat transfer procedures have advanced significantly over the past decade and now one can achieve long lasting improvement especially when the fat is harvested from an area near the recipient area. For more severe defects, two stages may be required; the first stage involves restoring volume and natural contour to the defect, followed by a second procedure to fine-tune any remaining surface irregularities. In addition, fat can be frozen and used if necessary in the future if some of the fat from the original transfer is metabolized.
