1. After reading the articles, darken the appropriate circles on the answer sheet on page 190 (or a photocopy). Each question has only 1 correct answer. 2. Review learning objectives on this page and complete the registration information and program evaluation on the answer sheet. 3. Send the answer sheet with your registration fee to: Lippin-cott Williams & Wilkins CE group, 2710 Yorktowne Blvd., Brick, NJ 08723. You will receive your certificate in four to six weeks. For faster service, include a fax number and we will fax your certificate within two business days of receiving your enrollment form. If you pass, you will receive your CE certificate of earned contact hours and an answer key to review your results. If you fail, Lippincott Williams & Wilkins gives you the option of taking the test again at no additional cost. All answer sheets for this test must be received by August 31, 2009.
