B.Z. is a 58-year-old man who experienced a large left middle-cerebral-artery (MCA) stroke. He was intubated immediately and, a few days later, developed such significant swelling that, to survive, he had to undergo decompressive craniectomy. Waxing and waning mental status, pneumonia, and a urinary tract infection complicated his hospital stay. Two weeks after the event, he was still unable to swallow, and the family decided to have a PEG tube placed. On discharge to the nursing home, he was awake but globally aphasic; he seemed to recognize his family members but did not follow commands or vocalize. He had a dense right hemiplegia. One year later, he is living at home with his wife; he walks with a cane and has started verbalizing a little-he says "no" quite emphatically, as well as "yes" and "honey."
