Optimal maternal and fetal outcomes are the targets of prenatal care, often starting during the preconception period and extending beyond birth of the infant. Avoidance of toxins during pregnancy is paramount to the proper development of the fetus and infant. Consequently, advice about smoking, alcohol, drugs and foods such as soft cheese and fish are often rolled into one message: avoid these items during pregnancy. The remainder of nutritional counseling is wrapped up in a few directives such as eat healthily, drink milk and take a prenatal vitamin. Messages about fish intake are complex, and require that a knowledgeable clinician take the time to fully explain the current recommendations to a patient who is able to understand nuanced information. Furthermore, with regard to food choices, patient preferences and behaviors are probably the most important determinant of nutrition regardless of advice from health professionals.
