摘要

Placement of the endotracheal tube per os may impede the clinician's ability to perform oral and maxillofacial surgical techniques. Anesthetic monitoring personnel may also be relieved if the endotracheal tube is in a position where it cannot be dislodged during manipulation of the oral cavity. Pharyngostomy endotracheal tube placement is particularly indicated when performing oral fracture repair. Providing a patent airway for delivery of general anesthesia and by-passing the oral cavity allows closed reduction of oral fractures based on occlusal assessment. This technique aids in fulfillment of the basic principles of oral fracture repair which include anatomic reduction and restoration of occlusion. Multiple intubation/extubation maneuvers are stressful on personnel and cumbersome for the clinician. Extubation to "check" occlusion does not insure maintenance of occlusion after re-intubation. Use of a pharyngostomy endotracheal tube alleviates the need for extubation during surgical procedures to assess occlusion and is described step-by-step.
