In this volume on neuroanesthesia and neurocritical care, we have assembled six articles on seemingly unconnected topics. However, despite the appearance, I believe they illustrate the broad spectrum of the knowledge base and the scope of practice of the neuroanesthe-siologisteurointensivist. The knowledge base spans from basic science with understanding of the anatomical basis of loss of consciousness with induction of anesthesia as demonstrated by functional imaging, as well as the effects of anesthetic agents on cerebral blood flow and metabolism (Drs Ramani and Wardhan; pp. 530-536) to clinical techniques and neuromonitoring. An increasingly sophisticated imaging paradigm has allowed us to peer into the site of action of anesthetic agents in a dynamic fashion that heretofore could not even be imagined, let alone studied. Although not addressed in this volume, other molecular imaging methods may some day help us to understand the actions of propofol on mitochondria that place patients at risk of arare but potentially fatal reaction to a widely used anesthetic, propofol (paper by Drs Cremer and Kalkman; pp.544-551).
