Chronic pain is estimated to affect over one-quarter of the world's population, and presents a considerable therapeutic challenge. This Nature Collection brings together articles from Nature, Nature Medicine, Nature Neuroscience, Nature Reviews Neurology and Nature Reviews Neuroscience that highlight recent advances towards understanding the risk factors and mechanisms that underlie chronic pain, and developing effective, non-addictive treatments for this highly prevalent condition. Opioids are frequently prescribed to treat chronic pain, but the safety of long-term opioid therapy remains uncertain, as its use is associated with several negative outcomes, including addiction. In a News & Views article in this Collection, Filizola and Devi discuss how recently published structural data relating to the ligand-bound conformation of opioid receptors might aid the development of new drugs that retain the beneficial effects of opioids while reducing their adverse effects. Another problem is the development of opioid tolerance, which renders these compounds ineffective. This issue is addressed by Wang et al., who found that blockade of the receptor tyrosine kinase PDGFR-beta in animals could eliminate morphine tolerance without inhibiting the analgesic effects of this drug.
