In a pregnant woman suffering from herpes zoster, various topical treatments for the lesions can be offered, including washing with soap and water, ice bags. For pain relief in pregnant women, paracetamol is the first choice drug, although its use should not be trivialised, given the uncertainties surrounding its potential long-term effects in children exposed during pregnancy. In cases of severe pain, morphine is an option, taking into account the risk of respiratory depression and withdrawal syndrome when taken close to delivery. Few data are available concerning the long-term psychomotor development of children exposed in utero. Treatment with an antiviral is only justified in case of ophthalmic involvement or disseminated zoster. Aciclovir is the antiviral drug which has been the most studied for the treatment of herpes zoster during the first trimester of pregnancy (with follow-up of about 6 000 pregnancies). No significant safety signal was detected, despite its action on DNA. During pregnancy or close to delivery, taking an antiviral exposes the child to the same adverse effects as the mother.The long-term effects are unknown. Use of valaciclovir, a metabolic precursor of aciclovir, enables the number of doses to be reduced compared to aciclovir, but less is known about its effects during pregnancy as it has been less studied. There are even more unknowns with fam-ciclovir due to lack of data.
