Career Development in Pediatric Critical Care: Are We Failing Our Trainees?

Most pediatric critical care medicine (PCCM) is practiced in academic settings, but increasingly com-munity hospitals are developing PICUs and hiring pediatric intensivists. Thus, some proportion of graduating PCCM fellows can be expected to pursue careers as clinicians in community hospitals. In a study published by Freed et al in 2011, approximately 19% of surveyed pediatric intensivists were noted to be practicing in community hospitals. Fellowship training programs, however, are housed in academic medical centers, and require 3 years of training, of which a significant proportion is dedicated to research or other scholarly activity. It should not be surprising, therefore, that Cifra et al found, through their survey of PICU fellows and recent graduates published in this issue of Pediatric Critical Care Medicine, that most fellowship programs do not provide career guidance for practice in a community-hospital based PICU. What is potentially more surprising, however, is the lack (or perceived lack) of preparation for administrative positions or leadership training. The authors' findings are not substantially different from a survey by the in-training section of the Society of Critical Care Medicine (SCCM) in which the second most commonly perceived need by critical care practitioners after continuing medical education was leadership development.
