For patients with ovarian cancer sensitive to platinum salts and a BRCA gene mutation, who have received at least two lines of platinum-containing chemotherapy, maintenance therapy with niraparib (Zejula, Tesaro) offers no clinical advantages over olaparib. Both drugs prolong the median time before requiring treatment with another cytotoxic drug by just over 1 year, with no demonstrated survival benefit, and frequently provoke sometimes serious adverse effects. The harm-benefit balance of niraparib appears unfavourable in the absence of BRCA gene mutations, having only been shown to increase the median time to treatment with another cytotoxic by 4 months.
