Nasopharyngeal decongestants are sym-pathomimetic vasoconstrictors.They can provoke cardiovascular events, including stroke and myocardial infarction (1).A German team has reported a striking case of a 60-year-old woman who felt that her heart rhythm had been irregular for 5 days (2). She was diagnosed with atrial fibrillation and cardioversion was performed, with resumption of sinus rhythm. Atrial fibrillation recurred 10 days later. She had been using a decongestant containing the imidazoline derivative tramazoline 5 times a day by nasal application for several years. Cardioversion was repeated and the decongestant discontinued, and she had no further recurrences of atrial fibrillation in 6 months of follow-up.
