In April 2020, the case of a patient admitted to hospital with a withdrawal syndrome after taking one tablet of nalmefene was reported in France,This opi-oid antagonist had been prescribed for alcohol de-pendence.The patient was also taking buprenorphine as substitution therapy for opioid dependence (1). This signal is reminiscent of other cases reported to French regional pharmacovigilance centres (CRPV) and to the French Centres for Evaluation and Information on Drug Dependence and Addictovigilance (CEIP-A), in particular 90 reports of nalmefene combined with an opioid, recorded in the French pharmacovigilance database between September 2014 and August 2017 (2). These reports consisted of combinations of nalmefene with methadone (46 cases), buprenorphine as opioid substitution therapy (27 cases) or as analgesic therapy (1 case), morphine (5 cases), fentanyl (2 cases), tramadol (2 cases), oxycodone, dihydrocodeine, codeine, or codeine + paracetamol. 71 cases (79%) had serious consequences, including 65 hospital admissions or life-threatening situations.
