Lamotrigine: very variable exposure in breastfed infants

A cohort study conducted in the United States, published at the end of 2019, has provided some data on exposure of in-fants to antiepileptic drugs via breast milk. Seven antiepileptics, including lamotrigine (Lamictal0 and other brands), were studied by taking a blood sample from the breastfed infants and their mother, at the same consultation, between 5 and 20 weeks after birth (1). 70 assays of lamotrigine plasma concentration were carried out in these mother-infant pairs. Lam-otrigine was detected in most of the infants (62, i.e. 88.6%), with a lower limit of quantification of 0.1 microg/ml.The lamotrigine plasma concentration varied between 1.4 and 9.9 microg/ml in the mothers (with a median of 6.2 microg/ml) and between 0.05 and 8.5 microg/ml in the breastfed infants (with a median of 1.6 microg/ml). The ratio between lamotrigine concentrations in infants and mothers was found to be highly variable, with extreme values in the infants ranging from 1% to more than 90% of the maternal plasma concentration (1).
