In mid-2019, the Regional Pharmaco-vigilance Centre in Nancy, France reported a case of stroke after consumption of a plant-based food supplement (Wiltrol) promoted as a sexual stimulant and purchased on the internet. The 50-year-old patient was admitted to hospital with severe hypotension, dysarthria, hemiparesis and left-sided facial paralysis. A brain scan showed a recent occipital ischaemic stroke, as well as middle cerebral artery stenosis.The following day, a further transient episode of cerebral ischaemia occurred, with prolonged hypotension (1). Two days before the onset of symptoms, the patient had taken one capsule of WiltroT. Toxico-logical analyses carried out on the capsules revealed the presence of 100 mg of sildenafil and 14 mg of tadalafil per capsule (i.e. amounts corresponding to high daily doses of these drugs).These vasodilators cause arterial hypotension, leading to a risk of stroke, particularly when used in combination (1-4).
