Various drug shortages have resulted in substandard patient care and adverse events, sometimes causing serious harm. Errors, some of which were fatal, have occurred when a different product is provided to replace the unobtainable product. The danger is particularly high with cytotoxic drugs and drugs with a narrow therapeutic index. The differences between an unobtainable product and its replacement that are most likely to lead to errors involve their bioequivalence, strengths or concentrations, and packaging. It is sometimes better to repackage the replacement drug. It is very important that healthcare professionals and patients report all adverse events related to drug shortages.
