The US Joint Commission on Accreditation of Healthcare Organizations has identified poor communication as the most common cause of wrong-site operations.1 However, the rarity of such events makes it difficult to evaluate the effectiveness of interventions.2 One of the consequences is the acceptance of surrogate end-points such as 'perceived risk' and 'perceived collaboration'. An example is the study carried out by Makary et al? who administered a case-based version of the Safety Attitudes Questionnaire to operating room (OR) staff at an academic medical centre, before and after initiation of an OR briefing programme.
