Wilson and Lewis emphasise the potentially overambitious hopes for primary care networks (Editorial, 14 September). Positive impact is plausible, but not guaranteed. Unintended consequences frequently occur. Clinical outcomes or patient experience don't necessarily improve and might deteriorate. Diseconomies of scale can emerge. It can take decades for large scale interorganisational collaborations to mature and function effectively. "Successful" collaborations often have focused and measurable goals and dedicated support.
