Both the National Institute for Health and Care Excellence (NICE) in the United Kingdom (UK) and the National Institute for Health (NIH) in the United States of America mandate that patients try and fail medical weight management before they can be considered for bariatric surgery. Such staged algorithms where the more invasive options are reserved as a last resort option exist for many other health conditions like coronary artery disease. It would indeed be appropriate to come up with such algorithms for management of obesity too if there were cost-effective alternatives to bariatric surgery that worked in the long-term. This does not, however, appear to be the case and long-term data on the success of medical weight management are scarce. In comparison, bariatric surgery produces durable weight loss. Success achieved with medical weight management is not only minimal in comparison with bariatric surgery but in the absence of long-term data, one suspects not lasting. Moreover, in an increasing environment of budgetary constraints, one cannot overlook the cost. Medical weight management can cost between ￡900.00 and ￡1250.00 per patient per year and the data on the long-term cost-effectiveness of such management are difficult to obtain. In comparison, the cost of bariatric surgery within the National Health Service (NHS) of the UK is approximately ￡5000.00 and the data in support of its cost-effectiveness compelling
