摘要

Fatigue is a common presentation with such a wide differential diagnosis that it can be difficult to streamline the workup. This challenging case is of a 76-year-old man presenting after 1 month of worsening weakness. Part of the workup focused on the patient's renal failure. Beginning with a urinalysis, a stepwise approach was used that led to exploration of a chronic, cellular-mediated autoimmune process. Ultimately a tissue diagnosis was need. Pauci-immune glomerulonephritis is a rare, treatable disease that fit our patient's profile. The specific diagnosis was essential to understand its natural history and to direct management.
