In this issue, Butler and co-workers provide one of the most comprehensive reviews to date of hyperbaric oxygen therapy (HBOT) for ocular indications. The retina has the highest rate of oxygen consumption of any organ, and many common eye diseases are associated with hypoxic-ischemic damage. Despite this, HBOT has neither been widely used nor appreciated in ophthalmology.The authors extensively review nine ophthalmic disorders for which the Undersea and Hyperbaric Medical Society recommends HBOT: decompression sickness and arterial gas embolism with visual symptoms, central retinal artery obstruction (CRAO), periocular gas gangrene, mucormycosis, periocular necrotizing fasciitis, carbon monoxide poisoning with visual symptoms, radiation optic neuropathy, radiation-induced scleral necrosis and periorbital reconstructive surgery. Other ocular indications for which significant, albeit less strong, evidence exists include: ischemic optic neuropathy (especially when associated with hypotension during surgery or dialysis), ischemic central retinal vein obstruction, branch retinal artery or vein obstructions associated with visual loss, and cystoid macular edema of post-operative, ischemic or inflammatory etiology.
