Could a transanal approach be the future treatment for a nonhealing colorectal anastomotic leakage?

Dear Editor, We read with interest the article by Dapri et al. [1] in which two anastomotic leaks occurring after rectal surgery were treated by a transanal approach with a reusable transanal D-port and 5-mm reusable monocurved instruments (DAPRI, Karl-Storz Endoskope). In both cases, anastomotic leakages were repaired by transanal procedures with simultaneous formation of a laparoscopic stoma. As is well known, the transanal approach is used for various indications, especially for down-to-up total mesorectal excision and polyp removal by transanal minimally invasive surgery [2,3]. However, this technique is not routinely used for the closure of colorectal anastomotic leakage. We want to congratulate Dapri and colleagues for the technique and for the invention of this new transanal platform and transanal instruments.
