Buiting and colleagues' study suggests that medical oncologists' treatment decisions in end stage cancer are based mostly on rational considerations regarding (side) effects. This contrasts with the findings ofourqualitative interview study with medical oncologists that explored the factors contributing to such decisions.All 17 doctors interviewed reported numerous "non-medical" factors that they deemed relevant for treatment decisions in end stage cancer. They cited personal (for example, own clinical experience), situational (for example, time pressure), and other contextual (for example, available resources) factors. Moreover, several doctors acknowledged that such decisions are not necessarily determined by patients' preferences but often reflect their own perceptions and values.
