摘要

The authors report a case of an 83-year-old man who had an eight-week history of right groin pain. Three weeks after the pain started, he was unable to walk due to limping. This was accompanied by fatigue and morning stiffness. After another two weeks, he developed loss of power in both arms and the upper legs. He had much difficulty in walking, climbing stairs, and dressing himself. Physical examination revealed tenderness in the right groin, but there was good hip and lumbar spine range of motion. Neurologic examination was normal. There was slight swelling of the left wrist joint, but there were no psoriatic lesions or signs of sacroiliitis. Normal red blood cell and white blood cell counts were noted, but the erythrocyte sedimentation rate [ESR] was elevated [70 mm]. Rheumatoid factor and antinuclear antibody titers were in the normal range. Ultrasound [US] of the groin revealed a large fluid collection [6 cm x 2.5 cm x 3 cm] lateral to the iliac vessels and adjacent to the psoas muscle. This was identified as iliopectineal bursitis. The bursa was aspirated, presumably with US guidance, and 7 ml of clear yellow fluid was obtained. The left radiocarpal joint was also examined by US, and a small amount of fluid was found in the radio-carpal joint. Examination of the aspirated fluid was normal [i.e. no crystals, no infectious agents, etc.].
