Dear Editor, Impairments in dual tasking are an early feature of Hunting-ton's disease (HD) that severely affect activities of daily living. Studies have documented impairments in walking during talking tasks (motor-cognitive), bimanual tapping tasks (motor-motor) and paired choice reaction and digit backward dual tasks (cognitive-cognitive) [1-5]. Despite the negative impact of such impairments on activities of daily living, only one study has explored the effect of a non-pharmaceutical treatment strategy on dual tasking in HD. This study found no improvements in dual tasking after 12 weeks of multimodal exercise in individuals with manifest HD [6]. Previous studies of Parkinson's disease (PD) [7] and mild dementia [8] have documented improvements in dual tasking after memory and dual-task training, respectively, although not demonstrated in HD. These findings suggest that exercise alone is not sufficient for improving dual tasking in individuals with HD and highlight a need for incorporating other therapeutic approaches, such as cognitive and dual-task training, as a multidisciplinary therapy strategy for improving dual-tasking performance.
