Treatment goals and plans should be made in conjunction with the patient and individualized based on disease severity and a patient-specific analysis of risks and benefits. Pharmacists are in a unique position to make recommendations based on patient-specific characteristics, promote medication adherence, and monitor for drug interactions and adverse effects.
