Objective: King County Emergency Medical Services (EMS) and community partners implemented a sobering center pilot to provide an alternative destination outside of the Emergency Department (ED) for individuals with acute alcohol intoxication in South King County, Washington. Methods: In addition to collecting client utilization data, we conducted key informant interviews, site visits, and surveys to identify program barriers, facilitators, and areas for improvement. Results: The sobering center served 87 unique individuals, many of whom were experiencing home-lessness, acute alcohol intoxication, and had complex health and social issues. Most encounters (64%) were self-referrals, and a small proportion of clients (13%) engaged with behavioral health services after their sobering center admission. Project partners identified the collaboration of partners and administrative support as key facilitators of the pilot, while also indicating design limitations and communication challenges.Conclusions: The sobering center's clients received continuity of care and brief interventions in the sobering center. Project partners identified the need for culture change among first responders and strong community partnerships to improve sobering services in the future. These findings can inform the integration of harm reduction services with other wrap-around, community-based, and patient-centered services for marginalized populations experiencing substance use disorders.
