An analysis of over 2000 medication errors reported in France involving children, more than 650 of which resulted in serious adverse effects, showed that they were predominantly wrong-dose or wrong-drug errors. The results of an analysis of reports of actual or potential medication errors in children were published in 2021. These reports had been recorded in the database of the French Health Products Agency's (ANSM) medication error reporting system between 2013 and 2017 (1). An error is termed "actual" when it reaches the patient, and "potential" when it is intercepted before the drug has been administered to the patient (2). More than 2000 errors in children over a 5-year period. Among the 4718 reports of errors that included the patient's age and stated whetherthey had occurred in a hospital or community setting, 2332 involved children (1). 791 reports were actual or potential errors that occurred in a hospital setting. 1541 reports concerned errors in non-hospital settings (in the home, residential care facilities, etc.) (1).
