Four Bartholin's gland carcinomas constituting 5.3% of vulvar cancer and illustrating the aggressive nature of this tumor are presented. A racial preponderance for Caucasians is mentioned. The pertinent literature is reviewed and recommendations consist of increased physician awareness, closer follow-up of benign disease, careful re-evaluation of unresponsive bartholinitis, frequent use of biopsy, excision rather than marsupilization of Bartholin's lesions in the elderly, and routine use of cytology at the time of marsupialization.
