Although prediction of fetal maturity prior to delivery is necessary in many cases, the ideal method has not yet been devised. In our experience, estimation of fetal maturity using the dye, Nile blue, to stain cells in amniotic fluid has been correctly interpreted in almost every instance. Presented in this study arc results from use of this assay in 110 selected cases. The patients include those with normal gestations as well as those with Rh disease, toxemia, diabetes, and sickle cell disease. Using birthweight, Dubowitz's criteria, and the absence of respiratory distress syndrome as determinants of fetal maturity, this method was more than 98% accurate (108 of 110). In addition, this assay was not affected by the appearance of blood, meconium, or vaginal secretions in the amniotic fluid. The accuracy of other methods for detecting fetal maturity in the presence of fetomatemal disease and other variables is also discussed.
