We appreciate the interest and suggestions expressed in the Letter to the Editor entitled "Intrathecal trastuzumab: What else do we need to consider?" by Gao et al. Indeed, our colleagues raise 2 relevant points about the role of the European Association of Neuro-Oncology (EANO) and European Society for Medical Oncology classification of leptomeningeal metastasis (LM) and the role of combined systemic and intrathecal treatment. Leptomeningeal metastasis remains a challenging diagnosis due to low sensitivity of cerebrospinal fluid (CSF) cytology and magnetic resonance imaging (MRI) assessments along with often unspecific clinical presentations. Per protocol, patients were eligible for the NCT01373710 study if CSF cytology demonstrated the presence of tumor cells or, in the absence of tumor cells in the CSF, if patients presented with typical clinical symptoms of LM in combination with typical findings on MRI. All patients had a histological proof of cancer and had a clinical assessment, a cerebrospinal MRI assessment, and a CSF cytological assessment, in line with the work-up recommended for the diagnosis of leptomeningeal metastases published in 2017, 4 years after the inclusion of the first patient in the NCT01373710 study.
