"You were lamenting the single key per lock in your LOTO lock station. This is actually by design. As you noted, OSHA has fairly strict rules for LOTO. One of them is that a company's LOTO procedure must ensure that nobody can unlock the lock of another employee while that employee could be vulnerable. When there are multiple keys available, somewhere, this can be problematic. As such, many companies (mine included) only allow one key per employee lock. That effectively ensures only the employee can remove the lock with a key (and although an employee could give his or her key to another, this is a serious offense here). Of course, other companies have different policies and do allow multiple keys. To your point: If an employee is gone from the premises, a lock sometimes may have to be cut. Employees aren't supposed to leave their personal locks on equipment when they leave here. We use special transition locks when something needs to be locked out between shifts. However, it happens - and when it does, it requires a committee of 3 people to ensure the employee is definitely not on premise and that it's safe to simply remove (cut) the lock - or remove the lock and replace it with a transition lock.
