People with substance abuse problems constitute the largest group of patients referred to the psychiatric emergency department. The purpose of this study was to compare users of ‘legal’ drugs to illegal drug abusers. During a 1-year period psychiatric emergency referrals for problems related to substance abuse were monitored in four public hospitals with a standardized checklist. The results showed, next to sociodemographic differences, a significant difference in the referral pattern of both groups. Illegal abusers were more self-referred and visited the emergency more often during office hours, without symptoms of intoxication or precipitating stressors. The response of clinicians was also quite different according to the nature of the drug: the psychiatric comorbidity of illegal substance abusers appeared to be underestimated, the information they gave was often mistrusted and they were referred much more often to outpatient facilit
