This review discusses basic research into the etiologic mechanisms of the adult respiratory distress syndrome (ARDS), as well as therapeutic strategies directed at mechanisms of disease and supportive strategies involving mechanical ventilation and hemodynamic management. The concept of O, supply/delivery in ARDS, and its possible contribution to the development of multisystem organ failure (MOF), is briefly discussed.
