This review will discuss a number of recent publications which examine the role of the various mechanical factors which may contribute to ventilatory failure and C02retention in patients with chronic obstructive pulmonary disease (COPD). In such patients the balance between the demands of breathing and the capacity to do so is highly unfavorable.
