Although first described over 100 years ago, the exact pathophysiology of pre-eclampsia remains unclear. Recently, much work has been carried out to identify some of the cellular mechanisms involved, and these will be reviewed, along with developments in therapeutics and intrapartum and postpartum management. The application of a better understanding of the underlying pathology, and the exquisite sensitivity of the critically ill patients will lead to more exact protocols for management.
