Primary sarcomas constitute a small but complex group of ovarian tumors. Among the 2400 cases in the Emil Novak Ovarian Tumor Registry, 43 have been diagnosed as primary sarcomas. These have been subdivided into three categoriesamp;mdash;teratoid, mesenchymal and mullerian (paramesonephric). The clinicopathologic criteria for differentiation as well as the features of prognostic significance are discussed. Although other areas of neoplasia in the upper genital canal have been associated with lesions of paramesonephric origin, such relationships are absent among the other types of primary sarcomas.
