Observations and treatment of women suspected of having broad ligament lacerations of varying degrees with resulting pelvic congestive disease syndrome are reported. Two groups of women, those in the perimenopausal stage and a younger group who have not reached that particular stage, were studied. The perimenopausal group was treated, as a rule, by hysterectomy. A new concept of the transverse repair of the fascial tear of the broad ligament in younger women, which has produced excellent results thus far, is described. Uterine phlebograms and celioscopy probably will provide more sophisticated diagnostic procedures prior to operation.
