This double-blind single-centre crossover study compared felodipine extended-release (ER) 5 to 10mg with nitrendipine 10 to 20mg, both once daily, with respect to antihypertensive efficacy and tolerability. 22 patients with a supine diastolic blood pressure (DBP) of 95 to 110mm Hg were randomised to 4 weeks of treatment each with felodipine ER 5mg once daily and nitrendipine 10mg once daily. If the supine DBP was 90mm Hg after 2 weeks of treatment, the doses were doubled. 24-Hour ambulatory blood pressure monitoring after 4 weeks of treatment showed that felodipine ER significantly reduced systolic blood pressure (SBP) and DBP compared with nitrendipine. Heart rate was unchanged. Clinical SBPsol;DBP measurements 24 hours postdose were lower with felodipine ER compared with nitrendipine; however, the difference was not statistically significant. Two patients did not complete the study because of vasodilatory effects. Both treatments were well tolerated.
