Drug monitoring was performed over the duration of long-term, high-dosage thiopental sodium therapy in 119 patients hospitalised for neurological damage. Total doses of 437.9 plusmn; 183.5 mgsol;kg for females and 310.8 plusmn; 124.9 mgsol;kg for males were given over 126.5 plusmn; 48.2 hours. For the whole group, the mean steadystate plasma thiopental sodium concentration was 21.5 plusmn; 10.3 mgsol;L and the corresponding clearance value (CLss) was 2.25 plusmn; 0.93 mlsol;minsol;kg. For each patient, the plasma concentration-time curve was plotted over the full period of data collection, according to a one-compartment model with linear elimination (96 patients) or (nonlinear) Michaelis-Menten elimination (23 patients). Linear and nonlinear groups exhibited no difference in demographic characteristics or in thiopental sodium treatment, but the total dose was statistically lower in males than in females. In patients with first-order kinetics, the apparent volume of distribution (Vd) averaged 3.12 plusmn; 1.76 Lsol;kg and elimination half-life was 18.3 plusmn; 11.0 hours. In patients with nonlinear kinetics, mean parameter values were 3.23 plusmn; 2.18 Lsol;kg for Vd, 63 plusmn; 19percnt; for saturation of the enzymatic system, 15.3 plusmn; 10.8 mgsol;L for the Michaelis constant (Km) and 15.3 plusmn; 10.8 mgsol;Lsol;h for the theoretical maximum rate of drug elimination (Vm). Only plasma concentrations at cessation of thiopental sodium treatment (Cstop) discriminated between linear and nonlinear models (19.0 plusmn; 10.6 and 27.2 plusmn; 15.7 mgsol;L, respectively).
