Posterior knee pain is a common symptom. Main etiologies include muscle, tendon, ligament, bone, meniscus, bursa, vessel or nerve pathologies. Posterior knee pain revealing focal osteoar-thritic (OA) lesions of the far posterior femoral condyle (FPFC) is unusual. Knee OA is a disease of the whole joint. Its pathogenesis involves systemic and local biochemical and biomechanical factors 1. The development of a focal cartilage lesion, in a non-weight bearing area, suggests specific biomechanical factors, such as focal chronic stress. Herein, we report clinical and radiological findings in 2 patients presenting with FPFC focal OA, and we discuss hypothesized pathogenesis. A 53-year-old woman and a 34-year-old man, with no medical history, experienced posterior mechanical pain of the right knee.
