Anti-thrombotic therapy for people with chronic systolic heart failure remains controversial. Although it is recognised that people with heart failure are at increased risk of embolic and thrombotic events, no study has found a clear benefit of anti-thrombotic agents in heart failure. Furthermore, there are concerns that aspirin may have harmful effects in people with heart failure and that aspirin may attenuate, or even negate, the benefits of ACE inhibitors by blocking the production of vasodilator prostaglandins through inhibiting cyclooxygenase.The Warfarin/Aspirin Study of Heart Failure (WASH) assessed the feasibility of conducting a large trial with one arm receiving no anti-thrombotic therapy.
